Student and Teacher Math Mindset Surveys (Printable and Google Forms)

Are you in need of a great way to track student or teacher mathematical mindsets over time? These math mindset surveys are a great way to get a sense of whether your students or teachers have a growth or fixed mindset. The student surveys are available in print versions and google form versions. The teacher survey is only available in google form version. There are 3 main types of surveys in this purchase:

K-2 Student Mindset Survey

Questions are presented in kid-friendly language. Pictures are provided to help students relate to their feelings/emotions.

3rd and Up Student Mindset Survey

Questions on this survey are slightly more complex and thought-provoking. Students are asked to self-reflect in a number of ways. There are more open response questions on this survey.

Teacher Mindset Survey

The teacher mindset survey is meant for coaches or principals to administer to their teachers anonymously to track their personal mindsets around math as well as their thoughts about their students’ math mindsets and habits. Questions about teaching style and structure are also asked to learn how their math class time is used.

Use these surveys to track mindsets on a micro level (one student or a small group of students), a class or grade level, or a school wide level. I hope you enjoy learning about the mathematical mindsets at your school.
